  2. PRINT & MAIL ONLINE FORM. Fill out the online form but print and mail (or fax) it to us instead of submitting it online. Or...

  3. DOWNLOAD, PRINT & FILL OUT PDF FORM by hand then mail or fax it. (Adobe Reader required).
